AFLAC SPECIAL INSURANCE:

The following special insurance is available to BOCES employees through AFLAC.  November is the open enrollment period.

5. The AFLAC Hospitalization Plan

Note: These policies roll over each November and do not need to be renewed on a yearly basis. November is the open
enrollment period for the next calendar year (January – December).

EAP Information

FLEXIBLE SPENDING PLAN:

Medical and Dependent Day Care Flexible Spending plans are available through HealthEquity. This plan gives you the option to use pretax dollars for dependent day care and medical costs. There will be 20 payroll deductions during the plan year; deductions will not be made during the months of July and August. November is the open enrollment period.

  • Unreimbursed Medical $3,050 Max/year.
  • Dependent Day Care $5,000 Max/year.

Note:  Enrollment in Flexible Spending does not carry over from year to year. You must fill out a new election form each November. The plan runs from January – December.

You may wish to consult your accountant regarding tax considerations before completing the Flexible Spending Plan Enrollment Form.

HEALTH SCREENINGS:

New York State Legislation was passed allowing public employees release time from work for breast and prostate cancer screenings. The release time is one allowance, not to exceed 4 hours, per fiscal year, July 1st to June 30th.

What to do:

  • Fill out a Personal Day form (See your supervisor or program secretary for the three-part form)
  • Check OTHER for the reason of absence and indicate Health Screening.

Please note: Documentation of attendance from your physician, after the screening, is required.

SICK BANK:

All full-time and part-time SSA unit members electing to participate in the SSA Sick Leave Bank shall have 31 days to enroll. Open enrollment is October.

SICK TIME – COVID REIMBURSEMENT:

To be reimbursed for sick days due to isolation/quarantine send a hard copy of the Department of Health Order of Isolation/ Quarantine along with the completed COVID Leave Approval Form, via mail or interoffice mail, to BOCES’ office of Human Resources.
